The only required document for the first visit to the Animal Clinic is the patient's passport. But try to take with you as complete a list of documents related to previously received consultations, diagnosis and treatment - this will help the specialist who conducts the reception at the clinic, get the most information about a particular case, and save time and money for pet owners.

Therefore, when visiting the Animal Clinic it is highly desirable to have the following documents:

  • extract from the medical history or medical card of a patient from another veterinary clinic;
  • data from previous instrumental research methods: ultrasound (ultrasound), radiography, CT (computed tomography), MRI, magnetic resonance imaging, endoscopic examinations, etc. Data can be stored on digital media (CD / DVD discs, USB - drives), as well as be printed on film and in paper form;
  • veterinarian's report indicating concomitant chronic diseases;
  • data of laboratory tests of blood, urine and other biological materials;
